Healthier You is a nine-month behaviour change programme that helps supports those at risk of developing Type 2 diabetes. Participants receive support to change their lifestyle in a friendly and supportive group environment.
Reed Wellbeing is delivering the programme to people largely referred by primary care, at a range of community venues.
People are diagnosed with Type 2 diabetes each year.
On referral, we conduct an individual assessment, including a BMI check and, where appropriate, very brief advice on smoking cessation.
Participants then attend thirteen 90 minute group sessions over 40 weeks. They receive extensive supporting resources, including handouts, trackers and a pedometer.

For those who are not able to attend our group sessions, they can access the service through a digital app provided by our partner Our Path.
More than 2,000 people are referred into our services every month.

Who are not pregnant at time of referral
Blood glucose level
In the past 12 months that indicates prediabetes (HbA1c 42 – 47 mmol/mol; FPG 5.5 – 6.9 mmol/l)
Not already affected
Those with no subsequent diagnosis of Type 2 diabetes
